London-based Consultant Ophthalmic Surgeon Mr Charles Claoué, reported the recent case of an implantation of a Rayner custom-made +42D Rayner C-flex® intraocular lens.

“Patient X, aged 57 came for refractive surgery knowing he was long-sighted having been refused by a high-street laser chain. He was aware of a right corneal scar and life-long poor vision. On examination, uncorrected visual acuities were <6/60 and BSCVA 6/18 right +15.75/-3@160 and left 6/18 +13.00/-1@125. Axial lengths were 19.08mm right and 19.16 left. There was 7 dioptres of right corneal plane astigmatism consistent with a nasal scar. HofferQ biometry gave IOL values of +36.5 right and +42.5 left.
After full informed consent, he underwent refractive lens exchanges with Rayner C-flex aspheric IOL implantation. These IOLs werecustom made. The right eye also underwent two 90 degree astigmatic keratotomies with per-operative pachymetry. These high-power IOLs were injected without any special difficulty through the standard Rayner disposable injector.
Post-operatively, the UCVA of the right eye was 6/24 and the left 6/18. The reduced vision in the right eye was due to residual astigmatism and the corneal opacity. The patient was fully satisfied.”

Mike Ring, Rayner’s Director of Operations commented, “Custom made intraocular lenses are not only for patients needing one of our new high diopter toric lens corrections or for a multifocal toric (M-flex® T), but very often these can also be for patients needingcustomised versions of our more regular lens designs. Since the 1950s, Rayner has a tradition of making IOLs to-order which lie outside our advertised power-range and size ranges. These are mainly for human applications but we have also made high power and specially-sized lenses for veterinary use. Ophthalmic Surgeons interested in ordering such a lens should ask for a quotation via iol_enquiries@rayner.com”.
